Deena Pourshaban has over a decade of experience partnering with more than 25 Federally Qualified Health Centers (FQHCs), Medi-Cal managed care plans, and health systems across California. She specializes in strengthening health center operations and increasing revenue while expanding access to care.
Deena is a practice transformation expert, enhancing design and oversight of health center operations programs and implementing specialty care. Deena supports organizations with achieving the National Committee for Quality Assurance鈥檚 (NCQA) Patient-Centered Medical Home (PCMH) recognition and advancing specialty care integration. She has led multiple large-scale initiatives, including the Enhanced Specialty Access Initiative, Health Center Controlled Network, Los Angeles Practice Transformation Initiative, and Moving Clinics Upstream.
Deena has guided over 10 community health centers to PCMH recognition. She also recently co-led the empanelment workstream for 32 community health centers participating in the Kaiser Permanente Population Health Management Initiative, implementing empanelment processes and workflows. Her current work includes co-leading the Enhanced Specialty Access Initiative鈥攁 collaboration between the Department of Health Care Services, Kaiser Permanente, and four FQHCs to expand specialty access for Medi-Cal patients. Additionally, she is part of a large-scale initiative supporting the San Francisco Department of Public Health to improve provider productivity, clinical operations, and financial performance across 18 FQHC sites. She is responsible for leading the Clinical and Operational workstream focused on improving productivity and performance.
Deena has a Master of Public Health in biostatistics and epidemiology from the University of Southern California and a bachelor鈥檚 degree in human biology from the University of California, San Diego.
